How do I apply for the SFC (Structured Family Care) Program?
To get into the SFC program, you first need to apply to either the CCSP or SOURCE Program. Once accepted into either one of those programs, upon your request, our volunteers will apply and transfer you to the SFC program.

Can I keep my doctor?
Yes! If you apply to the CCSP Program, you can keep your current physician. If you decide to apply for the SOURCE Program, as long as your doctor is on the list of physicians, you can keep your physician.
How long does the process take?
30-45 days for admission into the SOURCE and SFC program. The CCSP Program takes approximately 45-60 days.
Can I be a part-time caregiver to my family member under the SFC Program?
No. To be a caregiver under the SFC Program, you must be available full-time. However, under certain circumstances, under the SOURCE and CCSP Programs, you may be able to do so.
Can I be the caregiver for my child?
No. The SFC program does not allow a parent to be their child’s caregiver, a legal guardian to be the caregiver, or the spouse to be their spouse’s caregiver.
What is the eligibility for a Caregiver in the SFC program?
The family caregiver must be biologically or legally related to the care recipient. However, a spouse, legal guardian, parent of a minor child, or conservator is not eligible to be the structured family caregiver. The family caregiver must reside in the same home as the care recipient. The family caregiver is not allowed to have any other means of employment. The primary caregiver cannot be employed or self-employed.
How much does it cost for your home care services?
Factors that determine the cost of home care services are:
- Current medical condition of your loved one
- Level of assistance required to complete daily-living tasks such meal prep, bathing, running errands, etc.
- Home care services are available on an hourly, daily, weekly or live-in basis and can be increased or decreased based on your budget.
What types of in-home care services do you provide?
A One Home Care provides companion, personal, and skilled nursing care services. Our affordable home care services are customized to allow your family member or adult to live at home in familiar surroundings while relieving you of stress and worry.
Will I always have the same caregiver for my family member?
A successful home care experience for your loved one is based on consistent, reliable service by our caregiver. We aim to establish a trusted, long-term relationship with one caregiver for your family. If, for any reason, your loved one requests another caregiver, we’ll search our extensive inventory of personnel to find the perfect match. It may be necessary to schedule more than one caregiver if your loved one receives care for many hours in a day or many days a week. In any event, the goal is the same: to have happy patients and families. We’ll work with you to ensure the same goal is achieved every time.
